Dear Leah and team, You and your program are incredible. Your home visits, as well as your help navigating the system for supports from mobility aids to pain management, were incredible. We could not have kept my mother at home and comfortable without you, the physiotherapist, the nursing assessor etc. We had no idea how to navigate the system and you truly helped. My mother's was at home, exactly where she wanted to be for her last months, thanks to you and the team. To AHS management please keep the palliative team supported, they save you money but are there for patents when needed. What more could you want in a team?! Many thanks again, Mary Persson
